What do I need to access My Health Folder?
- Be 16 years or more
- You must go to your health center and make the request in person, proving your identity.
- To have an email address and a mobile phone to receive SMS.
- If you already have credentials for making the request above to access the web Personal Health Folder, you need not do it again.
