Aplicativo para coleta de dados da atenção básica para Agentes Comunitários de Saúde. Essa ferramenta vai ajudar o trabalho dos ACS que fazem parte das
Equipes de Saúde da Família (ESF) ou do Programa Agente Comunitário de Saúde (PACS).
- Utilize nosso app para realizar as Fichas de Visitas Domiciliares, Cadastros Domiciliar, Cadastro de famílias e Cadastro Individual.
- Utilize o seu CNS Profissional para habilitar o app, consulte no site oficial do Ministério da Saúde / DATASUS (MS):

- Permite também auxiliar identificação da Cartão Nacional de Saúde (CNS) / Cartão-SUS.
- Fichas no formato do eSUS AB (SISAB), padronizado com o Cadastro Nacional de Estabelecimentos de Saúde (CNES).
- Melhore o acompanhamento de Gestantes, Pré-Natal ou Puerpério, Hipertensos (Hipertensão Arterial), Diabéticos, Pessoas com Tuberculose ou Hanseníase, Crianças, Acamados,
Domiciliados, Idosos, além de contribuir nas campanhas nacionais de saúde ou do seu município.
